Social Worker - Palliative Care

We are seeking a Palliative Care trained MSW/ACSW/LCSW to join our thriving interdisciplinary team at Santa Rosa Memorial Hospital/Queen of the Valley Medical Center. The palliative care social worker is an integral member of the inpatient palliative care team and our ideal candidate will demonstrate clinical competence by providing evidence based palliative care interventions, utilizing their expert lens to provide patient care, and contribute to the palliative care team's process and wellbeing.

Exceptional candidates will have the ability to provide empathy, support, and compassion to our patients and fellow team members.

Location: Napa, CA

Work Schedule: On Call / Per Diem

Shift: 8-hour, Days; Occassional weekends

Job Summary:

Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families. Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Essential Functions:



  • Demonstrates competency in evaluating patients' and families' psychosocial needs.
  • Demonstrates competency in providing social work and counseling services to patients and families.
  • Demonstrates working knowledge of and referral to appropriate community resources.
  • Demonstrates proficiency in psychosocial education and leadership to hospice/palliative staff and volunteers and the community.
  • Has read, discussed and is able to carry out hospice/palliative policies and procedures related to elder abuse, child abuse, suicide, drug and alcohol abuse and confidentiality.
  • Is able to effectively identify, assess and support patient/families' bereavement/serious health needs.
  • Is knowledgeable about families' needs related to time of death planning and is able to provide them with support and information.
  • Demonstrates sound understanding of and commitment to hospice/palliative philosophy of care.
  • Demonstrates sound understanding of and commitment to hospice/palliative philosophy of team.
  • Enforces safety policies and procedures and safe practices by staff and volunteers.


Skills:



  • Skill in family counseling, psychosocial assessment and utilizing community and health care resources.
  • Skill in teamwork and communication.
  • Knowledgeable about different developmental needs of children and teens. (Hospice Only)
  • Knowledgeable of psychosocial issues and challenges confronting geriatric clients and patients.


Prior experience in the inpatient setting is desired along with a desire for continuous professional development to ensure we are offing the best possible palliative care experience to our patients.

Minimum Position Requirements:

Education: Master's Degree in Social Work, Psychology or Counseling or related field. Graduate from an accredited school preferred.

Experience: 1 year Experience in a health care setting or related field.

Licenses/Certifications:



  • California driver's license.
  • Current auto liability insurance.


Preferred Position Qualifications:

Education: Coursework/Training - Training in family counseling and case management.

Licenses/Certifications: LCSW
