We are seeking a Palliative Care trained MSW/ACSW/LCSW to join our thriving interdisciplinary team at Santa Rosa Memorial Hospital/Queen of the Valley Medical Center. The palliative care social worker is an integral member of the inpatient palliative care team and our ideal candidate will demonstrate clinical competence by providing evidence based palliative care interventions, utilizing their expert lens to provide patient care, and contribute to the palliative care team's process and wellbeing. We honor the unique and important role of the palliative care social worker and their commitment to whole person care as demonstrated by their commitment to ethics, biopsychosocial and spiritual assessment. Exceptional candidates will have the ability to provide empathy, support, and compassion to our patients and fellow team members. Location: Napa, CA Work Schedule: On Call / Per Diem Shift: 8-hour, Days; Occassional weekends Job Summary: Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families. Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving. Essential Functions:

Prior experience in the inpatient setting is desired along with a desire for continuous professional development to ensure we are offing the best possible palliative care experience to our patients. Minimum Position Requirements: Education: Master's Degree in Social Work, Psychology or Counseling or related field. Graduate from an accredited school preferred. Experience: 1 year Experience in a health care setting or related field. Licenses/Certifications:
Preferred Position Qualifications: Education: Coursework/Training - Training in family counseling and case management.
