Licensed Vocational Nurse Intake Assessment

Location: Napa, CA

Work Schedule: Full time - 80 Biweekly Hours

Shift: 8-hour, Days

The Intake Assessment Nurse LVN, under the supervision of the Clinical Supervisor or Operations Manager, acts as a contact for information and coordination of the referral processing for Hospice services from physicians, insurance companies, discharge planners and other outside referral sources. The Intake LVN provides access for new patients for Hospice services.

Required qualifications:

• Graduate of a LVN program.
• 1 year Nursing related work experience, demonstrating a working knowledge of clinical care practices and procedures.
• Currently licensed as a LPN/LVN in the State of California.
• Possesses and maintains current CPR Certification
• Valid drivers license with an automobile that is in good working order and insured in accordance with Hospice requirements.

Preferred qualifications:

• Community health/hospice, medical/surgical or case management experience.
