Job Description/Essential Functions:

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to assist them to live as independently as possible in a community based setting. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a qualified candidate interested in joining our Residential Service Group Home team at our location in Wilmington, Delaware.

The Residential Associate Clinician will provide coordination and guidance, counseling, and encouragement to participants as they transition from residential services to a more independent environment. The Residential Associate Clinician will also implement and document participant's progress in accordance with individualized rehabilitation plans and ensure a safe, healthy and secure environment which promotes growth and development. Some additional responsibilities would include the following:

  • Complete the initial and annual participant assessments and develop the participant's recovery plan from a person centered perspective.

  • Assist with the presentation in case conference for assigned participants.

  • Responsible for the care and maintenance of case records for all assigned participants.

  • Act as a liaison and consult with community agencies and families to maintain coordination in the recovery process.

  • Provide ongoing assessment of participant's mental illness symptoms and their responses to recovery to ensure that immediate and appropriate interventions are provided.

  • Participate in staff planning, training and internal/external administrative meetings.

Job Qualifications:
  • Bachelor's degree in counseling, psychology, social work, nursing, rehabilitation or other related human service field.

  • Knowledge of mental health and substance abuse systems and supports.

  • Ability to work independently.

  • Good communication skills both oral and written.

  • Possess a valid driver's license with an acceptable driving record.

  • Direct experience in mental health services with emphasis on direct care in a community based setting.

Scope:

  • Requires the ability to lift, carry, fold, bend, kneel, stack, stoop, which includes direct intervention with participants.

  • Frequent talking and listening.

  • Ability to climb and descend stairs.

  • Possess manual dexterity and fine motor skills.

  • Requires crisis intervention in emergency situations.

  • May be required to report to work during emergencies including inclement weather.
