Description
Locationsin Greensburg, Westmoreland County FUNCTION: Guides and directs residential staff on a wide spectrum of issues and concerns, related to the health, safety, and welfare of the individuals that reside in a Community Home setting. Provides staff support, coverage, assistance, and guidance during the designated weekend timeframes. Ensures compliance with Pennsylvania Chapter 6400 Regulations and the Pennsylvania Department of Human Services Office of Developmental Programsstandards. Reports to the Healthcare Manager. RESPONSIBILITIES
- Supports the achievement of the individual's personal outcomes.
- Assists in implementing the Residential Supports Department Policies and Procedures relative to Community Homes.
- Provides for adequate staffing levels at each home in accordance with each person's ISP. Works shifts providing direct support and administering medications as needed or assigned.
- Communicates and resolves issues and/or concerns with the Healthcare Manager. Informs the Community Homes Specialist and Community Homes Supervisor of any health, safety, and welfare issues. Communicates information via phone messages, email messages, and an on-call log of the weekend's issues and concerns.
- Communicates with the HCSIS Point Person scheduled for the weekend rotation. Ensures all reporting is done in the appropriate timely manner and is inclusive of all detailed information required.
- Manages potential crisis situations, should they occur, at the residential sites. Utilizes excellent judgment, listening, critical thinking, and assessment skills. Ensures that staff adheres to the appropriate procedures and that appropriate people, both professional and family have been notified.
- Maintains ongoing communication with the Healthcare Manager, Community Homes Specialist, and Supervisors regarding staffing and work performance of weekend staff.
- Provides direction to weekend staff based on multiple factors: the information provided, their knowledge of that particular individual, and the individual's specific medical, behavioral, and psychiatric protocols.
- Maintains an appropriate level of communication with family, guardians, day supports, advocates, and other support service personnel as directed.
- Communicates with medical personnel as circumstances require. Facilitates hospital discharges when appropriate. Coordinates any hospital admissions that occur. Ensures that all information is dispersed to appropriate parties. Reports all pertinent information to the HCSIS Point Person and Healthcare Manager.
- Ensures timely, efficient, and accurate response, reporting, and documentation of all issues and concerns communicated to them in their role as Weekend On-Call Coordinator/Scheduler.
- Participates on various committees and work groups as assigned.
- Maintains awareness of new developments and current trends in the area(s) of expertise.
- Receives 24 hours of training relevant to Human Services.
- Acts as a Designated Agency Person per the Medication Administration requirements.
- Performs other related duties as assigned. Qualifications
The equivalent of an associate degreein Human Services or Social Science. Previous experience in the field of intellectual disabilities preferred. Able to complete one-person transfers. Good interpersonal and organizational skills. Advanced decision-making skills. Must meet Act 33 and 34 requirements.
