Description

FUNCTION:

Provides direct care (i.e., face-to-face 1:1 unless the service description code indicates otherwise)services for people with disabilities in the home or community.Ensures the individual's health, safety, and well-being by assisting with acquiring, maintaining, and improving self-help, socialization, and/or adaptive living skills. Adheres toand implements established goals and outcomes assigned to the individual per the supports team. Reports to the Healthcare Coordinator. In the absence of the Coordinator, reports to the Home Care Supports Manager.

RESPONSIBILITIES:



  1. Supports the achievement of individual'spersonal outcomes and goals.
  2. Reviews and implements the individual's plan of care, both medical and, if applicable, behavioralprior to the beginning of each shift.
  3. Provides for or assists the individual in performing routine personal care tasks, adaptive daily living skills, and assistance in physical transfers and/or ambulation.
  4. Performs household tasks such as routine cleaning and laundry and bed-making, and monitors nutritional meal preparation as related to the care of the individual.
  5. Instructs and assists with tasks to maintain a healthy and safe environment for each individual. Continually practices universal safety precautions regarding infectious disease.
  6. Documents and maintains accurate records of implementation and progress towards the individual's goals and the activities in theircare each day on duty.
  7. Participates, either in person or via report, in annual, or as requested, support team meetings for the individual.
  8. Maintains a professional and cooperative working relationship with families and other team members.
  9. Informs the Health Care Coordinator/ Home Care Supports Manager in a timely manner matters relating to employment and/or certain issues regarding the care of the individual such as schedule changes, availability, hospitalizations, injuries, environmental issues affecting the health and welfare of the individual.
  10. Implements appropriate emergency procedures in the event of an emergency. Demonstrates and understands the responsibilities of reporting incidents per state and federal guidelines.
  11. Demonstrates an understanding of accreditation standards, federal, state, and county regulations as they apply to the care of the individual.
  12. Abides by all Achievaand Home Care policies and procedures.
  13. Maintains annual mandatory staff qualifications, Home Care training, and health clearances and
  14. Serves as a model of appropriate dress and behavior for the individuals supported.
  15. Attends in-house required mandatory provider meetings, at Achieva's corporate headquarters to secure program and/or regulatory updates, changes, and/or revisions.
  16. Performs other related duties as assigned. Qualifications

The equivalent of a high school diploma with one year of professional and/or personal experience as a provider of direct care support for persons with disabilities. Satisfactory ACT 33/34 and clearances required. Satisfactory FBI clearance is required depending on the individuals assigned. A valid Pennsylvania driver's license and a properly insured vehicle may be required.
