Housing Specialist Program Consultant- New Jersey

The Clinical Program Manager will work with the Housing Transition team. The incumbent will be responsible for the development and/or on-going management and administration of clinical programs. In this role, you will provide implementation support, measurement standards, and revisions as needed that incorporate applicable best practices with proven outcomes.

Expect to spend about 30-40% of your time in the field visiting our members. You'll need to be flexible, adaptable and, above all, patient in all types of situations.

Primary Responsibilities:



  • NJ Health Plan Housing Specialist - identifying and securing community-based housing for MLTSS Members whose barrier to transition is lack of affordable and accessible housing
  • Utilizing the state mandated contractual alignment process to partner with the Member's assigned Care Manager to ensure the development of a coordinated and seamless transition plan through engagement
  • Facilitating the Interdisciplinary Team consisting of NF/SCNF staff, Member and other providers or authorized representatives and caregivers as appropriate
  • Assigned protocol to assist the assigned Care Manager in assessing the Member's transitional needs, including the authorization for payment and procurement of transitional services. Including housing, furniture, property inspection and other required items as described
  • External Client/Member facing for interviews, missing data or correspondence for Member transition support
  • Monitoring support for the effectiveness of the NF to Community Transition Plan including performance measures that include: emergency room visits, hospitalizations, re-hospitalizations, NF admissions and other quality measures as required by the State to meet federal reporting requirements of the MFP demonstration

Required Qualifications:



  • Undergraduate degree in human/health services and/or behavioral health or equivalent experience (in healthcare data analytics)
  • 2+ years of case management/care coordination
  • 2+ years of program management/project management experience
  • Experience in a role supporting a quality function or related operational function
  • Experience extracting, trending and reporting data
  • Intermediate experience with MS Office including Word, Excel and PowerPoint
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance.
