The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of two main Divisions - the San Francisco Health Network and Population Health. The Maternal Child and Adolescent Health (MCAH) and Primary Care (SFHN PC) divisions are integral parts of San Francisco Health Network's complete system of care.

The MCAH Section carries out the administrative and operational components of the MCAH goals and objectives required by State and Federal mandates. MCAH has oversight and implementation responsibility for program guidelines with health providers and community agencies, and subcontractor compliance with the MCAH Scope of Work. MCAH mission is to provide quality care for birthing people, children and their families with a racial equity and trauma informed lens.

SFHN PC's fourteen (14) health centers/programs, provide high-quality, compassionate care to the City's children, youth, adults, and elders regardless of immigration status or ability to pay. Services include prenatal care and family planning services; integrated behavioral health and substance use services; dental, gender health, nutrition, pharmacy, and podiatry services; integrative health services such as acupuncture; and care for people experiencing homelessness and refugees/asylum seekers. We are a mission driven organization dedicated to eliminating heath disparities and becoming an anti-racist organization.

The SFHN MCAH/PC Physician Champion will have 54% administrative time to work in MCAH Section to provide clinical oversight for women's and adolescent health services within SFDPH, SFHN and MCAH. This role will also provide clinical care in one or more of the SFHN PC health centers/programs at 30% time. The balance of the position will utilize this direct service experience to champion equitable women's and adolescent health services within SFHN PC.

Job Description

Under the general direction of the Maternal, Child, and Adolescent Health Section Director, and in close collaboration with the Section's Family Planning and Perinatal Services Coordinators:

  • Provides clinical consultation to the Family Planning Program and Comprehensive Perinatal Services Program in their support of clinic-based family planning and reproductive health services at San Francisco Health Network and Comprehensive Perinatal Services Program provider clinics, including providing clinical consultation for:
    • QI efforts, including special projects and chart review tool development;
    • Staff trainings and brown bags;
    • Billing and medical protocols and workflows;
    • Improvement of EPIC/data management systems for family planning and pregnancy related services;
    • Ensure the above efforts are grounded in an anti-racist and adolescent and LGBTQQIA-inclusive framework
  • With the Family Planning Program and CPSP Coordinators updates clinics on:
    • New protocols, workflows, formulary, data management systems as per above;
    • New Family P.A.C.T., Title X, and CPSP regulations and guidelines;
    • Upcoming training opportunities and QI efforts;
  • Establishes and leads a citywide maternal mortality review process;
  • Participates in collaboratives working to improve sexual, adolescent, and women's health services within SFDPH/UCSF and the citywide hospital delivery system;
  • Develops and facilitates proctoring opportunities for other San Francisco Health Network and Title X clinicians to obtain and maintain medical privileges in IUD and contraceptive implant insertion and removal;
  • Ability to work with staff from diverse social and economic backgrounds with respect, understanding, and teamwork -- including and especially staff from non-clinical disciplines
  • Performs other related duties as assigned/required.

Under the general direction of the Primary Care Chief Medical Officer and Primary Care Director of Population Health and Quality, the Physician Specialist performs the following essential job functions within Primary Care:

  • Provides primary care, urgent care and relevant office based procedures to patients consistent with their medical specialty area through in-person, telehealth, and/or group visits
  • Provides telephone advice to patients
  • Provides patients with health education, counseling, and self-management support
  • Provides clinical case management for patients with complex care needs as indicated
  • Maintains accurate, timely and complete electronic medical records
  • Participates in staff meetings, project meetings, and conferences when indicated
  • Participates in quality improvement and quality assurance initiatives
  • Works effectively as part of a multidisciplinary team with Zuckerberg San Francisco General Hospital, MCAH, and community based organizations to coordinate care for patients
  • Provides consultative support to other members of the care team
  • Participates in education and clinical supervision of other staff members including Family Nurse Practitioners, Nurses, Medical Assistants and Health Workers, as well as UCSF medical students and residents who may rotate through the health center;
  • Champion women's and adolescent health services within SFHN PC with an equity lens
  • Performs limited administrative responsibilities and other duties as assigned by the PC CMO
  • Collaborates with identified project staff on special projects
  • Participates in after-hours remote on-call medical advice pool
  • Performs other related duties as assigned/required
Qualifications

Required

  1. Possession of a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California*

    AND
  2. Successful completion** of a residency program accredited by the Accreditation Council for Graduate Medical Education or American Osteopathic Association in the appropriate medical specialty area for the assigned facility or division (i.e., Board Eligible)

    AND
  3. Possession of valid Drug Enforcement Agency registration with the United States Department of Justice***

*Applicants possessing a valid license to practice medicine issued from another state within the United States of America may apply, but if selected, the candidate will NOT be appointed/hired until they obtain a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California.

**Applicants enrolled in a residency program may apply, but if selected, the candidate will NOT be appointed/hired until they demonstrate successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education or American Osteopathic Association in the appropriate medical specialty area for the assigned facility and obtain valid Drug Enforcement Agency registration with the United States Department of Justice.

Desirable

  • A minimum of three years providing family planning and perinatal health care within a primary care setting including to adolescents, including having proficiency in insertion and removal of long-acting reversible contraceptives.
  • Demonstrated commitment to racial equity and reproductive justice, including a minimum of twenty (20) hours in verified relevant training over the past three years.
  • Understanding of the tenets of racial humility, health equity, and reproductive justice
  • Desire to work with low income and multicultural clients, experiencing multiple, chronic medical and psychosocial challenges;
  • At least 1-year practicing in an outpatient setting in the past 2 years;
  • Two (2) years of experience in a community based or outpatient setting providing service to medically underserved populations in a multicultural environment;
  • Family PACT and CPSP billing knowledge and experience;
  • Interest in community primary care, managed care, quality improvement projects, teaching, and willingness to collaborate with other health professions and social service personnel;
  • Proficiency in Chinese, Korean, Spanish, or Russian;
  • Experience providing sexual health services, including in the areas of contraception, pregnancy options counseling, and STD testing and treatment, and perinatal services;
  • Proficiency in IUD and Nexplanon insertion /removal including clinical proctoring;
  • Board Certification in Adolescent Medicine and/or Family planning fellowship completion;
  • Eligibility and desire to obtain hospital birthing privileges;
  • Experience conducting clinical and/or staff training specifically for nursing, clerical front line staff on sexual health and perinatal topics;
  • Demonstrated ability to create and sustain relationships with community partners and institutional stakeholders
  • Experience developing clinical protocols and workflows;
  • Experience working in and/or developing a Trauma-Informed Care and/or Harm Reduction models
  • Training and experience in designing, implementing, and evaluating quality improvement initiatives including Lean Management Systems
  • Excellent organizational skills and analytical abilities
  • Demonstrated high attention to detail and ability to meet deadlines
  • Ability to manage multiple complex tasks concurrently
  • Experience with project management
  • Ability to work in a fast-paced, demanding environment
  • Ability to work both independently and cooperatively as part of multidisciplinary team

Conditions of Employment

The Department of Public Health (DPH) is implementing a unified Electronic Health Record (EHR) system and DPH employees must demonstrate competency in the use of the system that is appropriate for their classification as a condition of employment.

Personal Protective Equipment (PPE)

Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator's seal (i.e., weakens the seal betweenthe respirator and the wearer's bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.
