Case Manager RN - Remote with 10% travel in the callaway, audrain, or macon area

If you are located in Callaway, Audrain, or Macon area, MO, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:



  • Making outbound calls to assess members' current health status
  • Visiting members in the hospital to prepare for transition home (varies by market)
  • Identifying gaps or barriers in treatment plans
  • Providing patient education to assist with self-management
  • Providing clinic and provider education
  • Interacting with Medical Directors on challenging cases
  • Coordinating care for members
  • Making referrals to outside sources
  • Coordinating services as needed (home health, DME, etc.)
  • Educating members on disease processes
  • Encouraging members to make healthy lifestyle changes
  • Documenting and tracking findings
  • Making "welcome home" calls to ensure that discharged member receive the necessary services and resources

Required Qualifications:



  • Current, unrestricted RN license in Oregon or compact license
  • 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care
  • 2+ years of Case Management experience
  • Experience working with clients in the community setting
  • Demonstrate a proficiency in computer skills including Microsoft Word, with the ability to navigate a Windows environment
  • Designated workspace and access to install secure high-speed Internet via cable / DSL in home
  • Ability to travel up to 10% within a designated area
  • Access to reliable transportation
  • Currently resides in Oregon
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Preferred Qualifications:



  • Bachelor's Degree
  • Certified Case Manager (CCM)
  • 5+ years of experience as an RN
  • Experience / exposure with discharge planning
  • Background in managed care
  • Solid organizational skills and multitasking abilities will be keys to success
