Job Summary

Sentara Medical Group, part of the nationally-recognized Sentara Healthcare, has an opening for an experienced Nurse Practitioner to join Sentara PACE (Program for All-inclusive Care for the Elderly) in Norfolk, VA. PACE is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing facility eligible individuals who would prefer to remain at home with services coordinated by Sentara PACE. The program provides supportive care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation.

We seek a confident and independent Nurse Practitioner, with a minimum of three years' clinical experience as a licensed Provider who is passionate about care for the elderly. Hours are Monday - Friday and there is a very limited second-call responsibility of 1:3.

Qualifications:

MLD - Master's Level Degree: Nurse Practitioner (Required) Advanced Cardiovascular Life Support (ACLS) - Certification - American Heart Association (AHA) RQI, Authorization to Prescribe License - Certification - North Carolina, Authorization to Prescribe License - Certification - Other/National, Authorization to Prescribe License - Certification - Virginia, Licensed Nurse Practitioner (NP) - Certification - Other/National, Licensed Nurse Practitioner (NP) - Nursing License - North Carolina, Licensed Nurse Practitioner (NP) - Nursing License - Virginia, Registered Nurse (RN) Single State - Nursing License - North Carolina, Registered Nurse (RN) Single State - Nursing License - Virginia, Registered Nurse License (RN) - Nursing License - Compact/Multi-State License
