Job Description

JOB SUMMARY:

  • Assesses, plans, implements, teaches, evaluates, coordinates, supervises, and provides quality skilled nursing care to assigned patients of all ages under the direct order of a physician in accordance with agency policy and procedures. Supports System and Nursing Strategic Plans, Relationship-Based Care, and Shared Leadership.

ESSENTIAL FUNCTIONS OF THE JOB:

1. Evaluates and physically assesses condition and needs of patients of all ages on initial home visit and each re-visit.

2. Initiates, interprets and implements a patient plan of care for use of all disciplines. Updates plan as change in care develops. Modifies plan of care and goals with the written agreement of the physician at least bi-monthly and more often when indicated.

3. Provides skilled nursing care, utilizing accepted nursing and infusion therapy practice in accordance with all legal and regulatory standards, Agency protocols, policies and procedures.

4. Teaches and guides patient and family in prevention of further disease, demonstrates rehabilitative skills to obtain maximum performance in activities of daily living.

5. Assesses patient's compliance with therapeutic regimen and evaluates effectiveness of therapy. Realizes and promotes the spiritual opportunities available to the patient and family.

6. Communicates and coordinates with infusion companies, labs, physicians, and other healthcare providers concerning patient care; obtaining necessary physician orders, as needed.

7. Administers medication as required, following Agency protocols, policies and procedures.

8. Obtains laboratory specimens as required, transporting specimens according to Agency policy and procedure to limit exposure to blood and body fluid hazards.

9. Professional standards and current research/evidence-based practice drive decision-making.

10. Supports Shared Leadership and participates on unit-based team.

11. Coordinates all services in the care of the patient. Performs case coordination and discharge planning with all active disciplines, notifying appropriate staff of discharge, transfer, re-hospitalization and death.

12. Informs and consults with the physician regarding the patient's progress or regression to therapy/treatments ordered.

13. Observes records and maintains clinical records on all patients assigned. Adheres to Agency policy and procedures for the submission of required paperwork and management of documentation in the EMR.

14. Supervises the Home Health Care Aides working with patient a minimum of every 14 days.

15. Participates in bi weekly case conferences with the Clinical Supervisor.

16. Assists in orientation of new personnel to the Agency, acting as preceptor.

17. Serves as coach and mentor for less experienced colleagues.

18. Offers consistent and visible support to team members.

19. Communicates in an effective and professional manner, demonstrating respect for patients, families/significant others and other health team members.

20. Act in accordance with HEART behaviors and SRH values in your daily work and practice.

21. Responds to the following Shore Health System codes: Code Red (in the immediate area).

22. Responsible for honest behavior in all matters. To the best of the employee's knowledge and understanding, complies with all Federal and State Laws and Regulations.

23. Maintains the privacy and security of all confidential and protected health information. Uses and discloses only that information which is necessary to perform the function of the job.

24. Performs other duties as assigned for skill level.

Qualifications

MINIMUM EDUCATION, EXPERIENCE, LICENSE/CERTIFICATION REQUIRED:

  • Graduate of an accredited School of Nursing. Current RN licensure in the State of Maryland, or compact state. Minimum of one (1) year experience as an RN required, acute care preferred. Valid CPR. Valid Driver's License.
  • The employee in this position must be able to demonstrate the knowledge and skills necessary to provide care based on the age of the patients served. He/she must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status, to interpret the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the necessary care as described in the department's policies and procedures.
