Patient Navigator

Description Under the direction of the Program Director and Patient Coordinator Supervisor, you will provide high touch navigation for patients and family members on and off the UCLA Campus as designated by the Operation Mend program. You will work independently and with a team to coordinate all travel, hotels, and accommodations for Operation Mend patients and family members. You will escort patients and family members to and from the airport, clinical appointments, surgeries, events, and other locations as needed. You will accompany Operation Mend patients and family members into the designated appointments and serve as a liaison, and scribe to communicate with Operation Mend team members relevant information to optimize patient care and coordination. You will coordinate with care team to ensure that all transportation and escort needs are met for the Operation Mend Program for all clinical and nonclinical program activities. You must be flexible and excel in problem solving. Individual must be detail oriented and able to anticipate special needs for patients and other guests. You will proactively determine program needs and work with a team to create weekly and monthly schedules. You will maintain the highest level of professionalism and keep detailed and accurate records of all activities performed.
*Note: This contract position may convert to career.
Salary range: $22.03-$43.68 Hourly
Qualifications 1. Knowledge of military culture desired.
2. Experience working with patients with varying illnesses, injuries, and disabilities is desired.
3. Bachelor's degree preferred.
4. Working knowledge of BruinBuy and Concur desired.
5. Working knowledge of departmental policies and procedures on the safe transport of patients.
6. Knowledge of the UCLA Health landscape and medical campuses is strongly preferred.
7. Ability to establish and maintain cooperative working relationships with other staff members and all other personnel.
8. Skill in listening and comprehending information received in person or over the telephone to navigate and coordinate care for Operation Mend patients.
9. Ability to anticipate specific needs for individual patients, family members, and staff.
10. Detail oriented and ability to keep accurate and thorough records.
11. Ability to work standing, walking, and sitting for extended periods of time.
12. Interpersonal skills to interact diplomatically and sensitively with patients, staff, and visitors.
13. Working knowledge of Microsoft Office programs (Excel, Word, PowerPoint, etc.) is desired.
14. Ability to develop priorities, meet deadline, and maintain strong communication with the Operation Mend team
15. Ability to work independently on assignments with minimal supervision.
16. Must have a flexible schedule to include weekends and some evenings.
17. 1-year contract position, with the possibility to renew annually.
18. Maintain strong communication, time management and organization skills.
