What you will be doing

As of August 24, 2021, AmerisourceBergen requires all U.S. team members to be fully vaccinated and show proof of completed vaccine prior to your first day of employment. Shift/Hours: Must be able to work 11:00am - 8:00pm EST, Monday through Friday

Location: Remote, USA

PRIMARY DUTIES AND RESPONSIBILITIES:

* Conducts onboarding call to gather demographic information and assess patient and caregiver needs * Able to make decisions based on patient feedback and advises on appropriate resources available.

* Researches options based on Needs Assessment and provides care planning support Shares recommended care plan with patient/caregiver including patient follow up calls at a frequency determined by the patient and/or caregiver.

* Able to speak to client programs and make recommendations based on patient need. Educates patient on local advocacy chapters, events and community resources and aligning resources to patient/caregiver expressed needs.

* Acts as a liaison to facilitate 3rd party services such as nutrition, housing and transportation support. * Coordinates all Hub activities and key services including Access Counselors and Care Champions Communicates with internal FRMs as needed for Missing Information or HCP contact needs to ensure that the case continues towards completion

* Coordinates detailed activities with inpatient HCPs and mid level staff, HCP offices, outpatient facilities, patients and caregivers related to discharge planning including confirmation and communication of discharge plan and expectations, confirmation of first outpatient injection, prescription shipment coordination to appropriate outpatient setting for timely patient treatment in the outpatient site of care, etc.

* Provides exceptional customer service; resolves any requests in a timely and accurate manner and meets service level agreements.

* Effectively utilizes various means of communication, including but not limited to online, phone, fax, or mail methods

* Acts independently to determine appropriate recommendations of services available for patients.

* Performs related duties and special projects as assigned.

What your background should look like

EXPERIENCE AND EDUCATIONAL REQUIREMENTS:

Requires a 4 year college degree at an accredited university and broad experience in discharge planning within fields such as social work, nursing, counseling, psychology, discharge planning, or similar professions At least 2 years of directly related experience required.

* Licensure or certification in related healthcare discipline preferred Demonstrated Knowledge of both the financial and clinical aspects of care within various settings including transition between settings of care (e.g., inpatient to outpatient facilities including HCP offices, infusion centers, skilled nursing facilities)

* Experience working with interdisciplinary groups, hospital or out-patient setting preferred Knowledge and understanding of various payer and reimbursement models including commercial, Medicare and Medicaid with an ability communicate benefit information to HCPs, patients and caregivers.

MINIMUM SKILLS, KNOWLEDGE AND ABILITY REQUIREMENTS:

* Ability to communicate effectively both orally and in writing.

* Strong interpersonal skills.

* Ability to make quick decisions.

* Strong analytical skills. * Strong organizational skills; attention to detail.

* Ability to expertly use Microsoft Excel, Outlook and Word. Has wide-ranging experience, uses professional concepts and company objectives to resolve complex issues in creative and effective ways.

*Expert Knowledge of both the financial and clinical aspects of care within various settings including transition between SOCs

* Experience working with interdisciplinary groups, hospital or out-patient setting experience preferred

$56,400 - 80,630

*This Salary Range reflects a National Average for this job. The actual range may vary based on your locale. Ranges in Colorado/California/Washington State-specific locations may be up to 10% lower than the minimum salary range, and 12% higher than the maximum salary range.
