The Institute for Biomedical Informatics ( IBI) has contracted with the Cabinet for Health and Family Services, Department for Medicaid Services ( DMS) to create a team of licensed Registered Nurses that will perform assessments utilizing the Home and Community Based ( HCB) waiver assessment tool for the frail and elderly population and physically disabled. This position will identify, develop and present information educating professional staff, families and individuals seeking HCB Services while also making sound independent determinations as to the level of care and services needed for this vulnerable population.

This position can be located in Clark, Madison, or Estill County and is entirely remote. You will work out of your home and travel within your assigned counties to conduct in-home assessments. You will also have flexibility when scheduling in-home assessments to meet client needs. (Travel outside of the counties may be required on an as-needed basis.)

Knowledge of physical and psychological development. Excellent analytical and decision making skills. Ability to communicate effectively and interact with physically disabled and elderly individuals. Ability to travel throughout the state on a regular basis with the ability to travel out-of-state for overnight trips upon occasion.

Prefer experience working with the elderly and physically disabled. Driver's license is highly desired.
