This position is eligible for American Rescue Plan Act (ARPA) funds which includes a monthly stipend of up to $750.00 which is based upon the hours worked in the previous month. These funds are guaranteed through June 30, 2023. Case Manager Fast Facts
Child & Family's Aging Well Program has been providing vital support services to seniors in Newport County since the 1960s and to seniors in Providence since 2000. We support clients in obtaining resources to address their needs from housing to mental health services. Your team strives to enhance seniors' quality of life by focusing on preserving their dignity and respecting their independence. What You'll Be Doing As an Aging Well Case Manager working under Neighborhood Health Plan you will beproviding case management services to clients who are part of the Neighborhood Health Plan network. Case Managers provide in home visits, monthly calls, and assists with care coordination to ensure client needs are addressed.
