Salary Range: $38.25 - $57.65 / hour

The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Most often, a newly hired employee will be placed below the midpoint of the range.

Pay for PRN positions is calculated differently than Full-Time, Regular Part-Time, and Part-Time positions. PRN Rates depend on the position and other factors, including years of experience, or shift commitment according to the department policy. PRN differential is added in certain positions.


Position Overview:

The Palliative Care Social Worker addresses a client's physical, mental, social and spiritual well-being in all disease stages. Assessments focus on the goals, needs and strengths of both the patient and family caregivers. Educates patient/family, healthcare staff, and community on all aspects of palliative care and the services provided. Embodies and serves as an ambassador for the philosophy of evidence-based symptom management and whole patient assessment for patients with life limiting illness. Provides psychosocial support to palliative care patients at the Ventura and Ojai Campuses. Uses professional background and skills to assist the PC Team to facilitate patient/family meetings. Assists PC Team, attending physicians, staff, patients/families in the clarification of Goals of Care during hospitalization and the Plan of Care for post discharge. Ensures excellent patient care services through participation in education, outreach services and quality improvement initiatives. Links PC Team efforts with other hospital departments as well as outpatient community services to facilitate efficient discharge planning and clear follow up post discharge. Serves as a Community Liaison to ensure care coordination and comprehensive planning during the transition from the hospital or SNF setting.



    Master's Degree in Social Work or equivalent
  • Three (3) years direct patient care experience


  • Experience in internal medicine, geriatrics or oncology
  • Two (2) years Hospice/Palliative Care experience
  • Advanced certification in Hospice and Palliative Care
  • Bilingual in Spanish
