Description
POSITION: Family Resiliency Navigator

DEPARTMENT: Outpatient Therapy

Salary: $40,000 - $45,000 (Annually)

RESPONSIBILITIES:



  • Respond to requests for services and referrals to the program to determine needs.
  • Timely scheduling of intake to assess individual and/or family needs.
  • Direct and ongoing work with families experiencing social, emotional and mental health concerns.
  • Develop a Family Agreement and Action Plan based on Solution Based Casework for ongoing support.
  • Educate and teach individuals and families new skills to meet unmet needs.
  • Assist the family in cultivating natural and informal family and community supports
  • Continually identify and assess risk factors for children and families served and develop or update written safety plans/ action plans as necessary and appropriate.
  • Provide crisis intervention as needed.
  • Coordinate with team therapists to refer within the program for therapy when an opening comes available based on level of need.
  • Coordinate any supportive services and resources that children and families may need within or outside of the agency.
  • Continually analyze and assess each family and child situation on an individual basis, using advanced knowledge in the area of child welfare to develop recommendations regarding supportive services and resources that each child and family may need, such as educational plans, medical, psychiatric and psychological assessments, therapy, independent living skills, etc.
  • May participate in service planning meetings with the child, family, and/or school personnel
  • Respect the children and family's culture and provide culturally sensitive services.
  • Maintain a flexible schedule to meet the children and family's needs which may include working after traditional hours and on weekends.
  • Be accessible via email or cell phone to children, families, and other Cornerstones of Care staff in accordance with Cornerstones of Care on-call policy.
  • Attend all required training in accordance with Cornerstones of Care, and/or assigned by Cornerstones of Care supervisory staff.
  • Maintain and update all required information, including but not limited to: contacts made, monthly summaries in Cornerstones of Care information systems in accordance with Cornerstones of Care policy.


QUALIFICATIONS: This position requires:



  • Knowledge of computers including Word, Excel software and data entry.
  • Current driver's license, good driving record, vehicle insurance and five years of driving experience required.
  • Knowledgeable of and follow Cornerstones of Care policy and procedures, including social work code of ethics.
  • Knowledge of trauma-informed practices and knowledge of community resources.
  • Interpersonal Skills/Teamwork: Work cooperatively and maintain professional relationships with co-workers and clients/customers, to promote a harmonious work and team environment.
  • Contribution to Corporate Culture: Make positive contribution to corporate culture by supporting overall mission, vision, and values of Cornerstones of Care. Follow policies and procedures and work to accomplish goals and objectives of assigned program(s)/department(s), and agency. External Customer Service: Establish productive relationships with external people and organizations. Promote program and mission to clients, referral sources, and potential external supporters.
  • Ethics and Diversity: Adhere to agency code of ethics and professional standards. Show sensitivity and respect for cultural and ethnic diversity of clients, staff, and other community members and groups.
  • Attendance and Punctuality: Regular and reliable attendance is an essential function of this position.
  • Demonstrates and practices leadership skills
  • Must be at least 21 years of age and pass background check, physical and drug screening. This position also requires a valid driver's license and proof of current vehicle insurance
