We are searching for a RN-Care Manager to manage and coordinate care for SHN patients across the continuum. Collaborate with physicians, care teams, Inpatient and Post-Acute providers to achieve goals and enhance patients experience. Monitor and provide appropriate utilization management to achieve desired quality outcomes. Develop care plans to establish patient centered goals, monitor outcomes and drive engagement in comprehensive care management programs designated for high risk and at-risk members. Provide functional or technical supervision over other employees in same or closely related classifications, where scope of supervisory responsibility is limited to direction, guidance, explanation, etc. This position also requires meeting patients either in their home, provider's office, or in the community.



  • Hours: 40hrs
  • Shift: Day/Evening shift
  • Location: Southcoast Business Center - Fairhaven, MA
  • Travel: Driving on average 3 or more times per week required


Education and/or Experience Requirements:



  • A Bachelors Degree in Nursing or other health care field or the equivalent knowledge and experience is required.
  • If hired, must apply for and receive RI license if you do not have one.
  • As part of a multidisciplinary team, candidates must possess a strong teamwork ethic.
  • Candidates must also possess a current, unrestricted MA and RI nursing license.
  • Knowledge of accountable care is strongly preferred.
  • Advanced clinical experience, TMP or Medicare Advantage knowledge is preferred.
  • Certification in Case Management and case management experience is preferred.
  • Required skills include demonstrated ability to work with physicians to create care plans, advanced understanding of geriatric population, customer service and strong interpersonal skills are required.
  • Excellent organization, creative problem solving and communications skills are also required.
  • The ability to foster team work and computer competency within a Windows environment are required.
  • Proficient skills in Microsoft Office, i.e. Word, Excel, Access, PowerPoint are required.
  • Five (5) years related experience in an acute and/or other health care setting required.
  • Excellent interpersonal skills and strong negotiation skills are required.
