Come join our dedicated and caring team of professionals as a HIV Benefits Specialist. With guidance and direction from our Common Ground program Case Management Supervisor, the HIV Benefits Specialist will perform a full range of engagement services to people living with HIV including benefits screening, enrollment, management and troubleshooting. In this position you will also:

  • Provide exceptional trauma informed customer service to all internal and external customers and practice de-escalation techniques to reduce the risk of harm to patients and staff.
  • Provide non-medical case management support to the benefits clients; encouraging them to participate in Common Ground community activities.
  • Maintain supportive professional relationships with patients to support medical recommendations for viral load suppression.
  • Document interactions in the electronic medical record and record data in the County database, as well as run a monthly report for the County.
  • Participate in programmatic meetings including quality management and the team huddles.
  • Perform other duties as assigned by management.

Salary range: $30.53 - $40.30 Hourly

Qualifications

Required:

  • CA ADAP Enrollment Certification
  • Bachelor's degree in social work OR equivalent combination of education and experience.
  • Knowledge of HIV/AIDS, community resources, health education methods, advocacy, harm reduction and general clinical knowledge.
  • Skill in managing multiple deadlines/demands, and ability to follow through on assignments.
  • Ability to complete required program documentation, and skill in creating flyers and other program-promoting documents.
  • Ability to create and maintain boundaries with clients, and effectively interact with people and develop positive relationships while being tactful, respectful, and direct in communication.
  • Excellent verbal and written communication skills, and public speaking skills.
  • Ability to work independently and as part of a group; skill in maintaining cooperative working relationships.
  • Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality.
