Medication Reconciliation Technician - Pharmacy

We are looking for a Medication Reconciliation Technician for the Pharmacy Department at Santa Rosa Memorial Hospital.

Location: Santa Rosa, CA

Work Schedule: Full Time - 80 Biweekly Hours

Shift: 10-hour, Evenings

Job Summary:

Prepares medications (oral solids, injectables, IV admixtures, chemotherapy, etc.) for dispensing. Works under the direct supervision of a staff pharmacist to accomplish delivery of patient medications, utilizing personnel and automatic dispensing machines.

Essential Functions:



  • In coordination with Pharmacy management provides input for pharmacy technician performance evaluations.
  • Assists in developing and maintaining orientation and ongoing training program for pharmacy technicians and pharmacy students.
  • Maintains expertise in all pharmacy technician procedures and acts as a mentor and source of knowledge for less experienced technicians.
  • Assures the pharmacy and IV additive areas are maintained properly and in a clean/aseptic and orderly manner in accordance with Pharmacy Policy and Procedures.
  • Establishes and meets individual departmental goals which are developed to assist the Hospital in achieving established institutional goals.
  • Prepares new medication orders to be sent to nursing station including expediting STAT orders. Observes all legal and procedural steps to ensure final check of appropriateness by a pharmacist prior to dispensing.
  • Uses aseptic technique in laminar and vertical flow hoods, prepares large and small volume IV admixtures, prepares chemotherapy in vertical flow hood demonstrating appropriate handling technique for cytotoxic medications as required, prepares Total Parenteral Nutrition solutions, prepares sterile syringes as required.
  • Transport new medication orders to each nursing station, as required; pull and deliver floor stock medication to each nursing station or nursing station's automated dispensing device.
  • Communicates well and in a courteous manner with all personnel.
  • Enters and retrieves data into/from the pharmacy information system.
  • Answer phone calls in a polite manner and process any request. Refer questions requiring professional judgment to the pharmacist.
  • Restock medication trays, such as crash carts and other floor stock medication trays.
  • Extemporaneously prepare and package dosage forms into unit dose packaging observing all rules and record keeping requirements.
  • Purchase medications and other supplies as necessary for efficient operations. Assist with inventory control.
  • Complete at least one special project per year as assigned by Pharmacy management.
  • Diligently seeks and records information regarding all of the patient's home medications from family or directly from the patient in a manner that is professional and non-invasive.
  • Collects and compiles a complete and accurate list of patient's home medications including over the counter drugs and herbal supplements including drug name, strength, dosage, route, frequency, and last dose taken in a timely manner.
  • Interviews patient and family to obtain any known drug allergies and reactions.
  • Consistently follows up with community pharmacies and physician offices to develop an accurate home medication list.
  • Demonstrates initiative and focused attention when reviewing all patient home medication lists for accurate and complete information (as requested by physicians/nurses) within the first 24 hours of admission.
  • Collaborates with all members of the health care team to review the patient's medication list for any discrepancies including duplications, omissions, interactions or name/dose/route confusion.


Skills:



  • Pharmaceutical calculations.
  • Typing/data entry skills.
  • Drug brand and generic names is important.
  • Special requirements for the handling and disposal of hazardous drugs.
  • PC based computer systems.
  • Excellent written and verbal communication skills with people with diverse backgrounds and at all levels within the medical center and in the community.
  • Ability to create and foster a cooperative teamwork environment.
  • Basic computer skills.
  • Knowledge of commonly used prescription drugs including brand/generic names, available dosages, frequencies and indications.
  • Meets Santa Rosa Memorial Hospital's Standard of Behaviors including interpersonal communication and professional conduct expectations.
  • Preferred: Knowledge of medical terminology.
  • Preferred: Bilingual.
  • Preferred: Knowledge of Pharmacy operations.


Minimum Position Requirements:

Experience: Experience as a Pharmacy Technician.

Licenses/Certifications:



  • Registered as a Pharmacy Technician with the California State Board of Pharmacy.
  • National Certification (PTCB).
  • Current registration with the California State Board of Pharmacy.


Preferred Position Qualifications:

Education: Coursework/Training - Technician training course.

Experience: Pharmacy experience.
